MACRA Explained: How Medicare Rewards Quality Care Over Quantity

 

What is MACRA?

MACRA stands for Medicare Access and CHIP Reauthorization – Act of 2015. It is a U.S. law that changed the way doctors get paid by Medicare. Before MACRA, doctors were mostly paid based on how many services they provided, more tests, more visits equals more money. This was known as volume-based payment. MACRA changed this. Now Medicare pays doctors based on how well they take care of patients, not just how many services they perform. This is called value-based payment.

Why Did the Government Create MACRA?

Before MACRA, the old system had a big problem. A rule called the Sustainable Growth Rate (SGR) constantly threatened huge pay cuts for doctors. This caused uncertainty and frustration. Also, doctors had to follow several separate reporting programs, which made things confusing and time-consuming.

MACRA solved these issues by:

  • Ending the SGR formula – Stopped unpredictable Medicare pay cuts, giving doctors stable and predictable payments.
  • Combining old reporting programs – Merged multiple confusing programs into a single system to make reporting easier for providers.
  • Rewarding doctors for quality care – Incentivizes doctors to focus on patient outcomes and efficient, high-quality treatment.
  • Encouraging the use of modern technology like EHRs – Promotes electronic health records to improve record-keeping, communication, and patient safety.

How MACRA Works: The Two Pathways

MACRA created a new system called the Quality Payment Program (QPP). Under QPP, doctors can choose one of two ways to participate:

1. MIPS – Merit-based Incentive Payment System

In MIPS, doctors earn a score based on four things:

  • Quality (How well they care for patients)
  • Cost (How efficiently they use medical resources)
  •  Promoting Interoperability (How well they use electronic health records)
  • Improvement Activities (How they improve processes and patient care)

Depending on their score, doctors may receive:

  •   A bonus
  •  A penalty
  •  Or no change in their Medicare payments

2. Advanced APMs – Advanced Alternative Payment Models

This option is for doctors who want to join special care models that focus on teamwork and efficiency, such as:

  • Accountable Care Organizations (ACOs)
  • Patient-Centered Medical Homes

Doctors who join these models might take on financial risk, but they can also earn extra rewards, like:

  •  A 5% bonus
  • Automatic exemption from MIPS

Benefits of MACRA

  • MACRA helps improve healthcare by:
  • Encouraging better-quality care
  •  Reducing unnecessary tests and procedures
  •  Supporting the use of modern technology
  •  Making Medicare payments more predictable
  •  Promoting coordinated and patient-centered care

Challenges of MACRA

Even though MACRA is helpful, it isn’t perfect. Some challenges include:

  • Doctors must spend time reporting data
  •  Smaller clinics may find it harder to keep up
  • Some medical specialties fit the program better than others

In simple terms:

MACRA being the umbrella and MIPS & Advanced AMPs being the major components within it.
